NHotate / Mullein

Yet another variant of HNotate.

I've had second thoughts about the stateful document combinators that the recent HNotate variants were using. Particularly, they get rid of type safety making composition somewhat unsafe. Mullein is another HNotate version that aims for a more typeful style.
