Copperbox revision 751.

I've simplified the type classes in AbcOutput and LilyPondOutput and as a whole the types and their type classes obligations now seem right. Tomorrow I'll add a note type with fingering annotations, this should go a good way to seeing if I've hit a sweet spot where the types are right and flexible enough to handle extensions.
